Managing polychronic and frail elder populations is challenging, costly work. It requires intensive care management, longitudinal data, and multidisciplinary interventions.
GuideWell, parent of Florida Blue, realized it couldn’t do all that on its own, and so it joined forces with Alignment Healthcare, a population health management company, to better manage the care of its Medicare Advantage members. In 2016, the organizations opened six senior-focused clinics in Florida. The clinics feature a technology-enabled, patient-centered model of care, designed to reduce hospitalizations and improve clinical outcomes for medically needy elderly patients.
In this video, captured at the 2016 Oliver Wyman Health Innovation Summit, Rene Lerer, MD, president of GuideWell and Florida Blue, and John Kao, CEO of Alignment, discuss how and why the partnership works. They also say collaboration – not disintermediation – is the disruptive wave of the future, and the key to reining in health costs.
“We literally have the best doctors, best hospitals, best technology in the world. The idea is to assimilate and make those organizations better, not to displace or replace them,” Kao says.
